Abstract. The printing industry in Indonesia is one of the priority sectors. the growth of the printing industry today in Indonesia is still in the positive category. PT. Teenagers Rosdakarya is a company engaged in publishers and printing, products produced by the company are textbooks, fiction books, and non-fiction and books ordered directly by consumers. Production activities from 2013 to 2017 turned out to experience many product defects. The company has determined that the product defect tolerance limit is 0.40% but in reality defective products exceed these limits so that the company is detrimental. The type of defect that often occurs in the company is that the type of defect is peeled off the glue and the page is not suitable. As a result, such product quality will affect the value of a company and can lose the opportunity to get consumers. One that can be done by a company is that it must make improvements to reduce the number of product defects and find out the causes of product defects. The method chosen to be able to solve the above problems, then we need a method that can reduce the level of damage and repair it. The method used to identify the cause of the disability used is the Fault Tree Analysis (FTA) method and for the proposed improvement using the Failure Mode and Effect Analysis (FMEA) method and 5W + 1H.Keywords: Fault Tree Analysis (FTA), Failure Mode and Effect Analysis (FMEA)Abstrak. Industri percetakan di Indonesia merupakan salah satu sektor yang prioritas. pertumbuhan industri percetakan saat ini di Indonesia masih dalam kategori positif. PT. Remaja Rosdakarya merupakan perusahaan yang bergerak dibidang penerbit dan percetakan, produk yang dihasilkan oleh perusahaan yaitu buku-buku pelajaran, buku fiksi, dan non fiksi serta buku yang dipesan langsung oleh konsumen. Kegiatan produksi dari tahun 2013 sampai dengan tahun 2017 ternyata mengalami banyak kecacatan produk. Perusahaan telah menetapkan bahwa batas toleransi cacat produk yaitu sebesar 0,40% namun pada kenyataannya produk cacat melebihi batas tersebut sehingga merugikan perusahaan. Jenis cacat yang sering terjadi di perusahaan yaitu jenis cacat lem terkelupas dan halaman tidak sesuai. Akibatnya kualitas produk yang seperti itu akan mempengaruhi nilai suatu perusahaan dan dapat kehilangan kesempatan medapatkan konsumen. Salah satu yang dapat dilakukan perusahaan yaitu harus melakukan perbaikan untuk mengurangi jumlah kecacatan produk dan mengetahui penyebab-penyebab terjadinya kecacatan produk. Metode yang dipilih untuk untuk dapat memecahkan permasalahan diatas, maka diperlukan suatu metode yang dapat mengurangi tingkat kerusakan dan memperbaikinya. Metode yang digunakan untuk mengidentifikasi penyebab terjadinya kecacatan yang digunakan yaitu metode Fault Tree Analysis (FTA) dan untuk usulan perbaikan menggunakan metode Failure Mode and Effect Analysis (FMEA) serta 5W+1H.Kata Kunci: Fault Tree Analysis (FTA), Failure Mode and Effect Analysis (FMEA) |
