Abstract. PT. Mizan Grafika Sarana is one of the subsidiary units in the printing line of the Mizan Group which handles the production process of making books with 2 types of regular books and non-regular books, the research is focused on non-regular types of books. The purpose of this research is (a) to find out the types of disabilities that occur in the non-regular book production process at PT. Mizan Grafika Sarana, (b) identify the dominant factors causing the non-regular book production process at PT. Mizan Grafika Sarana and (c) provide suggestions for improvements to reduce defects in non-regular book products. The method used in this research is the Six Sigma method and the Failure Mode And Effect Analysis (FMEA) method. The Six Sigma method consists of 5 stages, namely Define, Measure, Analyze, Improve and Control. Define is used to identify / determine defects of non-regular book products. Measure is used to measure how much influence income has on the company. Analyze is used to analyze to find the causes of defects in non-regular book products. Improve is used to provide suggestions for improvements that will be made to the company. Control is used to compare how effective is the implementation of Define and Improve. The Failure Mode And Effect Analysis (FMEA) method is used to determine the greatest priority value of the defects that occur. The way to find out the priority value of disabilities that occurs is by determining the greatest likelihood of occurrence of disability (Severity), how often the disability occurs (Occurrence) and how often the causes of disability occur (Detection). Based on the results of research using the Six Sigma method, it is obtained a calculation of 3.54-sigma. Achieving the target towards 6.00-sigma still requires an increase of 2.46-sigma, if the disability is getting bigger and there is no significant action against the company, it can harm the company.Keywords: Six sigma, FMEA and Printing Industry.Abstrak.  PT. Mizan Grafika Sarana merupakan salah satu unit anak perusahaan pada lini percetakan dari Mizan Group yang menangani proses produksi pembuatan buku dengan 2 yaitu jenis buku reguler dan jenis buku non-reguler, penelitian difokuskan pada jenis buku non-reguler. Tujuan penelitian yang dilakukan adalah (a) mengetahui jenis-jenis kecacatan yang terjadi pada proses produksi buku non-reguler di PT. Mizan Grafika Sarana, (b) mengidentifikasi faktor-faktor yang dominan menjadi penyebab pada proses produksi buku non-reguler di PT. Mizan Grafika Sarana dan (c) memberikan usulan perbaikan untuk mengurangi cacat pada produk buku non-reguler. Metode yang digunakan dalam penelitian adalah metode Six Sigma dan metode Failure Mode And Effect Analysis (FMEA). Metode six sigma terdiri dari 5 tahap yakni Define, Measure, Analyze, Improve dan Control. Define digunakan untuk mengidentifikasi/menetapkan kecacatan produk buku non-reguler. Measure digunakan untuk mengukur seberapa besar pengaruh pendapatan pada perusahaan. Analyze digunakan untuk menganalisis mencari penyebab terjadi kecacatan pada produk buku non-reguler.  Improve digunakan untuk memberikan usulan perbaikan yang akan dilakukan pada perusahaan. Control digunakan untuk membandingkan seberapa efektif antara pelaksanaan Define dengan Improve. Adapun metode Failure Mode And Effect Analysis (FMEA) digunakan untuk mengetahui nilai prioritas terbesar dari kecacatan yang terjadi. Cara untuk mengetahui nilai prioritas kecacatan yang terjadi dengan cara menentukan kemungkinan terbesar terjadinya kecacatan (Severity), seberapa sering terjadinya kecacatan (Occourence) dan seberapa sering munculnya penyebab terjadi kecacatan (Detection). Berdasarkan hasil penelitian menggunakan metode Six Sigma didapatkan perhitungan sebesar 3,54-sigma. Pencapaian target menuju 6,00-sigma masih diperlukan peningkatan sebesar 2,46-sigma, apabila kecacatan semakin besar dan tidak adanya tindakan yang signifikan kepada perusahaan, maka dapat merugikan perusahaan.Kata kunci: Six sigma, FMEA dan Industri Percetakan.
