dc.description Abstract. Products produced by CV. Elleven is a travel pouch bag, carrier, weistbag, daypack and drybag. August 2018 to August 2019, daypack bag products were the most produced in the amount of 7087 pcs and had the highest average percentage of defects compared to other products at 6.02%. The average value of the percentage of defects exceeds the tolerance limit of 5%. There are 4 types of defects in daypack bag products, namely deviated stitching defects, untidy embroidery, scratches on materials and distorted labels. The FTA identifies the cause of the defect starting from the top event to the basic event. After knowing the causes of the defects, repair can be done using TRIZ. Basic events in deviating stitches, improper embroidery and distorted labels are fatigue operators, chasing production targets, less skilled operators, poor sewing needle quality, high machine use intensity, less machine maintenance, less air circulation and less lighting, whereas in defects scratches on the material are less cleanliness awareness, operator is not careful operator is less thorough, less raw material control, less ventilation and lighting. The proposed improvements to the cause of the defect are making From the Daily Product Quality Report and increasing supervision of the operator, providing visual control, making from operator skills, replacing needles with better quality and filling the needle replacement form, setting the machine according to SOP, performing machine maintenance regularly and filling from checking machines, checking and filling from raw materials, installing LED lights on sewing machines or production floors and adding facilities in the form of additional ventilation and installation of air-conditioning devices such as blowers in the production area. Abstrak. Produk yang dihasilkan oleh CV. Elleven adalah tas travel pouch, carrier, weistbag, daypack dan drybag. Bulan agustus 2018 sampai agustus 2019, produk tas daypack merupakan yang paling banyak di produksi yaitu sebanyak 7087 pcs dan memiliki rata – rata persentase cacat tertinggi dibandingkan dengan produk lainnya yaitu sebesar 6,02%. Nilai rata – rata persentase cacat tersebut melebihi batas toleransi yaitu sebesar 5%. Terdapat 4 jenis cacat pada produk tas daypack yaitu cacat jahitan melenceng, bordir tidak rapih, goresan pada bahan dan label menyimpang. FTA mengidentifikasi penyebab terjadinya cacat mulai dari top event sampai basic event. Setelah diketahui penyebab-penyebab cacat maka dapat dilakukan perbaikan menggunakan TRIZ. Basic event pada cacat jahitan melenceng, bordir tidak rapih dan label menyimpang adalah operator kelelahan, mengejar target produksi, operator kurang terampil, kualitas jarum jahit kurang baik, intensitas penggunaan mesin tinggi, perawatan mesin kurang, sirkulasi udara kurang dan pencahayaan kurang, sedangkan pada cacat goresan pada bahan adalah kesadaran akan kebersihan kurang, operator tidak hati – hati operator kurang teliti, kontrol bahan baku kurang, ventilasi dan pencahaya kurang. Usulan perbaikan terhadap penyebab cacat tersebut adalah membuat From Laporan Kualitas Produk Harian dan meningkatkan pengawasan terhadap operator, memberikan visual control, membuat from keterampilan operator, mengganti jarum dengan kualitas yang lebih baik dan mengisi form penggantian jarum, melakukan setting mesin sesuai SOP, melakukan perawatan mesin secara teratur dan mengisi from pengecekan mesin, melakukan pengecekan dan mengisi from bahan baku, memasang lampu LED pada mesin jahit ataupun pada lantai poduksi dan menambah fasilitas berupa penambahan ventilasi dan pemasangan alat pendingin ruangan seperti blower pada area produksi.
