Abstract. PT. LM is a company that engage in the field of stationary and produce pencil. In this company, there are problems with products that have defects, with seven types of defects, namely lead condition, torn wood, cracked pencils, rough pencil surfaces, paint/gloss peeling, even colour of paint, and stamping quality. In PT. LM itself, the implementation of Six Sigma as quality control method is relatively new. Whereas this method has advantages in solving existing problems in the company with the DMAIC framework (Define, Measure, Analyze, Improve, Control). The framework includes identification up to the resolution process of problems in the company to achieve quality improvement at PT.LM. At the Define stage (D), SIPOC diagrams are identifies, specifically by identifying the production process and production sequence whilst also identifies the people involved in quality improvement using Six Sigma. In the Measure phase (M), key criteria are determined using Pareto diagrams, by creating control charts and measuring the sigma level of the company. At the Analyze stage (A), and analysis is performed using a causal diagram supported with Failure Mode and Effects Analysis. In the Improve stage (I), repairs are made using 5W + 1H.The result of the Failure Mode and Effect analysis (FMEA) in the company shows that the defect was caused by the human-error on setting the machine, bad performance from the operator caused by exhaustion, lack of quality on raw materials, lack of color quality, lack of maintenance on the machine, dirt on the machine, the machine is stuck, the plastic mold is folded, less pressure on the pressing machine, the period of rubber replacement, and lack of application on SOP. Proposed recommendation for the company is to conduct periodic training to improve operator skills, the supervisor must oversee each  operator very closely, and determine the period of rubber replacement.Keywords: qualities, Six Sigma, failure mode and effect analysis, pencilAbstrak. PT.LM merupakan perusahaan yang bergerak pada bidang alat tulis yang memproduksi pensil. Pada perusahaan ini memiliki permasalahan pada produk yang mengalami kecacatan dan terdapat tujuh jenis kecacatan yaitu kondisi lead, kayu tercabik, pensil retak, permukaan pensil kasar, cat/gloss terkelupas, kerataan cat dan kualitas stamping. PT.LM sendiri, penggunaan Six Sigma merupakan metode pengendalian kualitas terbaru dimana metode tersebut memiliki kelebihan dalam memecahkan masalah yang ada dalam perusahaan dengan adanya kerangka DMAIC (Define, Measure, Analyze, Improve, control) serta kerangka tersebut meliputi identifikasi hingga penyelsaian masalah pada perusahaan untuk mencapai perbaikan kualitas pada PT.LM. Pada tahap define (D) dilakukan identifikasi diagram SIPOC, identifikasi proses produksi dan urutan produksi dan mengidentifikasi orang-orang yang terlibat dalam peningkatan kualitas dengan menggunakan Six Sigma. Pada tahap measure (M) dilakukan penentuan keriteria kunci dengan menggunakan diagram pareto, membuat diagram peta kendali dan mengukur level sigma pada perusahaan. Pada tahap Analyze (A) dilakukan analisis dengan menggunakan diagram sebab akibat dan analisis dengan menggunakan failure made and effects analysis. Pada tahapan improve (I) dilakukan perbaikan dengan menggunakan 5W+1H.Hasil Failure Mode And Effect Analysis (FMEA) pada perusahaan menyatakan kecacatan disebabkan oleh operator salah setting mesin, operator kelelehan, kurangnya kualitas bahan baku, kurangnya kualitas warna kurang baik, kurangnya perawatan pada mesin, adanya kotoran pada mesin, mesin macet, cetakan plastic terlipat, pengepresan kurang penekanan, waktu pergantian karet dan kurangnya penerapan SOP. Usulan yang dilakukan pada perusahaan adalah dilakukannya pelatihan secara berkala untuk meningkatkan keterampilan operator, pihak supervisor melakukan pengawasan kepada setiap operator dan menentukan waktu pergantian karet.Kata kunci: kualitas, Six Sigma, failure mode and effect analysis, pensil
