Abstract. Perimenopause is a period of transition from the reproductive phase to the old age phase (senium) due to decreased sexual function of the ovaries. Conditions that arise in perimenopausal women are associated with a decrease in the hormone estrogen. The International Menopause Society (IMS) recommends the use of Hormone Replacement Therapy (TSH) to treat the symptoms and signs of perimenopause. The aim of this scoping review is to understand the effect of giving the role of hormone replacement therapy on changes in symptoms and signs of perimenopause. This research is a Scoping Review, by searching for articles from the Pubmed database, Springer Link, and Science Direct. The number of articles that matched the inclusion criteria was 449 and the exclusion criteria were 443 articles. Overall results based on the PICOS criteria were 6 articles. This study was conducted in the period March-December 2020. The results of the analysis of six articles, 2 articles stated that the combination of estrogen and placebo had no effect on signs of perimenopause. 3 articles stated that giving combined estrogen, conjugated estrogen, and progesterone can reduce symptoms of perimenopause. 1 article using the hormone therapy estrogen or without progesterone can increase the symptoms of perimenopause in the form of sleep problems. The conclusion of the analysis carried out in this study is that administration of low doses of oral conjugated estrogen combined with transdermal estradiol and progesterone can relieve symptoms of perimenopause, whereas combined estrogen and placebo have no effect on signs of perimenopause.Keywords: Symptoms, signs, perimenopause, hormone replacement therapyAbstrak. Perimenopause merupakan periode peralihan dari fase reproduksi menuju fase usia tua (senium) akibat menurunnya fungsi generatif dari ovarium. Kondisi yang muncul pada wanita perimenopause berkaitan dengan penurunan hormon estrogen. Internasional Menopause Society (IMS) merekomendasikan penggunaan Terapi Sulih Hormon (TSH) untuk menangani gejala dan tanda perimenopause. Tujuan dari scoping review ini adalah untuk memahami pengaruh pemberian peran terapi sulih hormon terhadap perubahan gejala dan tanda perimenopause. Penelitian ini merupakan Scoping Review, dengan mencari artikel dari database Pubmed, Springer Link, dan Science Direct. Artikel yang sesuai dengan kriteria inklusi sebanyak 449 dan kriteria eksklusi 443 artikel. Hasil secara keseluruhan berdasarkan kriteria PICOS terdapat sebanyak 6 artikel. Penelitian ini dilakukan pada periode Maret–Desember 2020. Hasil analisis enam artikel terdapat, 2 artikel menyatakan pemberian estrogen kombinasi dan plasebo tidak memiliki efek terhadap tanda perimenopause. 3 artikel menyatakan pemberian estrogen kombinasi, conjugated estrogen, dan progesteron dapat mengurangi gejala perimenopause. 1 artikel dengan pemberian hormone therapy estrogen atau tanpa progesteron dapat meningkatkan gejala perimenopause berupa masalah tidur. Kesimpulan dari analisis yang dilakukan penelitian ini adalah pemberian dengan dosis rendah oral conjugated estrogen yang dikombinasikan dengan transdermal estradiol dan progesteron dapat meredakan gejala perimenopause, sedangkan estrogen kombinasi dan plasebo tidak memiliki efek terhadap tanda perimenopause.Kata kunci: Gejala, tanda, perimenopause, terapi sulih hormon |
